Job Description

Case manager to work with the three waivers we cover: Community Choice, HIV and Ventilator. Must be able to work with a computer, have a good telephone voice. Must be able to work unsupervised. Must have at least a BA in Social Work, Psychology, or one of the other social sciences.with two years of experience in the following area: service planning and assessment. Must have SC driver license, and an insured automobile. Must be able to successfully pass a SLED check. Salary discussed at interview
