Position Summary
The Case Manager provides comprehensive care coordination, advocacy, and education to pregnant and postpartum women and their infants (up to 18 months old). This role supports the
Missouri Bootheel Healthy Start Program
by addressing medical, social, and environmental needs during pregnancy and the postpartum period.
The Case Manager collaborates with clients, healthcare providers, and community organizations to ensure access to prenatal/postpartum care, early childhood services, and support systems designed for rural populations.
Service Region:
Butler, Dunklin, Mississippi, New Madrid, Pemiscot, and Scott Counties
Key Responsibilities
Provide case management services via telehealth and in-person visits, including assessments, care planning, service coordination, and follow-ups.
Facilitate access to prenatal, postpartum, and pediatric healthcare by coordinating referrals to OB/GYNs, pediatricians, WIC, lactation consultants, behavioral health providers, and other relevant services.
Conduct home visits and telehealth sessions to assess family needs and deliver education on:
Prenatal care and nutrition
Breastfeeding and infant feeding options
Postpartum recovery and mental health (e.g., postpartum depression screening)
Safe sleep practices, car seat safety, and child development milestones
Connect families with community resources to address barriers such as transportation, housing, food insecurity, and health insurance enrollment.
Advocate for clients within healthcare systems and community programs to ensure their needs are met with dignity and respect.
Build strong relationships with rural community partners to promote collaborative care.
Maintain accurate, timely, and confidential documentation in compliance with agency standards and HIPAA privacy requirements.
Participate in program evaluation and quality improvement initiatives to improve maternal and infant health outcomes.
Qualifications
Education:
Bachelor's or Associate's degrees in Social Work, Public Health, or a related field preferred.
Some college coursework preferred; relevant experience may substitute for education requirements.
Experience (Preferred):
Minimum of 2 years in case management, maternal-child health, public health, or a related field.
Knowledge of the Service Region's geography and demographics.
Experience working with pregnant women, infants, and families.
Familiarity with administrative duties in non-profit, healthcare, or social services settings.
Licenses/Certifications (Preferred):
Licensed Social Worker (BSW) or equivalent.
Certification in Case Management (CCM), Community Health Worker (CHW), or Maternal Child Health a plus.
Physical Requirements:
Ability to sit for extended periods and lift objects up to 35 lbs.
Skills & Competencies
Strong knowledge of prenatal and postpartum health, infant care, and community resources.
Excellent interpersonal, communication, presentation and advocacy skills.
Ability to work independently and manage a caseload effectively.
Proficiency in web-based software, data entry, and Microsoft Office applications.
Work Environment & Travel
Work performed in both office and community settings.
Local travel to attend meetings and program events.
Must have a valid driver's license, reliable transportation, and proof of insurance.
