Pivot Path Solutions is currently seeking three exceptionally talented Case Manager - Registered Nurse (RN)s to join our growing team to support a local project team.

Specific Tasks:

Provide these services in support of all service members who are seen and who are identified as having a medical/surgical or mental health issue when processing through for mobilization/deployment or demobilization/redeployment for any contingency operations. These shall be essentially the same duties as those required of any Medical Corps Officer or Government civil service provider of similar experience and in similar duty assignments. The Contractor registered nurse case manager shall have sufficient training/experience to be able to perform the duties of a case manager, to include, but not limited to the following:

  • Interview the service member, review the service member\xe2\x80\x99s medical record and coordinate requested consultations and medical follow up as prescribed by the providers.
  • Accomplish necessary coordination(s) to ensure that service members undergoing mobilization/demobilization receive access to medical appointments within 72 hours of request.
  • Follow up with service members once appointments are completed, to ensure that the service member is medically ready for deployment. If the Soldier is found medically ready for deployment, the service member is returned to their unit.
  • Use of telecommunications and information technologies to provide health assessment, diagnosis, treatment, consultation, education, and health-related information across distances.
  • Provide care services to the local MTF as needed.
Minimum Qualifications:

Degree: Associates Degree in Nursing.

Education: Graduate from a college or university accredited by the Accreditation Commission for Nursing Education (ACEN)

Certifications: Possess and maintain current Basic Life Support (BLS) certification. The American Red Cross or American Heart Association Basic Life Support for Healthcare Providers is the only program accepted. BLS recertification shall be maintained in a current status at all times while the CP is performing services under this contract. Possess one of the following certifications:
  • Commission for Case Manager Certification Certified Case Manager (CCM)
  • Certification of Disability Management Specialists Commission: Certified Disability Management Specialist (CDMS)
  • Association of Rehabilitation Nurses: Certified Rehabilitation Registered Nurse (CRRN)
  • American Board for Occupational Health Nurses Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S)
  • National Board for Certification in Continuity of Care: Advanced Certification in Continuity of Care (ACCC)
  • Commission on Rehabilitation Counselor Certification: Certified Rehabilitation Counselor (CRC)
  • American Nurses Credentialing Center Nurse Case Manager (RN-NCM)
  • National Academy of Certified Care Managers: Care Manager Certified (CMC)
Licensures: Possess a current unrestricted registered nurse license from any one of the United States of America, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands.

Experience: Possess a minimum of one year of experience within the last two years working as a full-time RNCM.

Citizenship: U.S. citizenship

Credentialing: Ability to pass background check and drug screen. Current physical and immunizations.

Preferred Qualifications:
  • Previous experience working with the military population.
