Responsibilities:
Conduct client assessments and identify care needs.
Develop and monitor individualized care plans.
Coordinate services across healthcare, social services, and community supports.
Advocate for clients and families.
Maintain timely, accurate documentation.
Communicate professionally with clients, caregivers, and providers.
Qualifications:
Required:
Case management, care coordination, or social services experience
Strong documentation and organizational skills
Ability to work independently and manage time effectively
Reliable internet and phone access
Preferred:
A bachelors degree from an accredited college or university in a health or human services field or related field.
At least 2 years of providing case management services
Mandatory in-person training must be completed in Columbia, SC prior to the official start of the position.
