JOB TYPE:

ESSENTIAL DUTIES AND RESPONSIBILITIES
:
1. Conduct initial assessments to identify the unique needs and strengths of each youth.
2. Develop personalized support plans in collaboration with youth, their families (if applicable), and multidisciplinary team members.
3. Coordinate access to essential services, including medical care, counseling, educational support, and legal assistance, based on identified needs.
4. Advocate for youth rights and access to resources within the organization and with external service providers.
5. Provide ongoing case management and support to youth, including regular check-ins, goal monitoring, and crisis intervention as needed.
6. Collaborate with the House Manager, Assistant House Manager, Therapist and Program Director to ensure consistent and effective care delivery.
7. Maintain accurate and up-to-date documentation of client progress, service coordination efforts, and outcomes.
CERTIFICATE/LICENSE/REGISTRATION:
1. CPR/FA Certification
2. Current driver's license, clean driving abstract, access to reliable transportation and current car insurance.
EDUCATION AND/OR EXPERIENCE:
1. Associate's Degree in social work, counseling, psychology, or the equivalent coursework in a related specialized field two (2) years of related work experience.
2. Previous experience working with youth who have experienced trauma, abuse, or homelessness.
3. Strong assessment, case management, and crisis intervention skills.
4. Knowledge of child welfare systems, relevant laws, and regulations.

Pay: $22.00 - $25.00 per hour
