Schedule: Full-Time - 8:30 AM - 5:00 PM, with one required late day per week (11:00 AM - 7:30 PM)

What You'll Do

As a

Case Manager

, you'll play a vital role in guiding patients through their recovery journey by:


Providing

case management and care coordination

services for clients in the facility. Coordinating

continuum of care activities

to ensure optimal resource utilization and compliance with referral sources. Conducting

psychosocial assessments

and facilitating individual, family, and group sessions. Managing treatment and discharge planning activities. Developing, implementing, and evaluating

personalized care plans

. Collecting and correlating clinical data for the treatment team and insurance providers. Serving as a liaison between the treatment team and external agencies to ensure seamless care.

What We're Looking For

Strong organizational and communication skills. Ability to work collaboratively with a multidisciplinary team with a minimum case load of 30 clients Minimum of 3 years' Experience in behavioral health or substance use disorder preferred. Minimum of a bachelor's degree in social work, Counseling, Criminal Justice, or a related Human Services field.
