Job Description

We're seeking a dedicated Case Manager / Family Consultant to support children in therapeutic foster care. In this role, you'll provide guidance, stability, and continuity of care by working closely with youth, foster families, and community partners. A Day in the Life

You will be responsible for assessing the individualized needs of any child placed in our care and their families by making regular contact, arranging for the provision of services, and maintaining documentation of actions and outcomes. You will focus the efforts of all team members, composed of the client in care, the birth family, the therapeutic foster parents, the custodial agency case worker, etc., on common goals and strategies to assure continuity of care across key persons and settings impacting the client's development.



How This Position Contributes to our Mission

Provide oversight of implementation of the treatment plan and client's progress through weekly contact, including but not limited to, two monthly face-to-face visits with the Therapeutic Foster Parent and client, weekly phone calls or emails to check in, and participation in quarterly treatment planning. Respond to client crisis in a timely manner to stabilize the client, encourage growth and learning for the client and foster parent, and mobilize needed resources to maintain client placement. Provide 24-hour on-call crisis intervention services (on a rotating basis). Receive and review monthly documentation from foster parents for adherence to meeting the needs of children placed with them as well as regulatory standards. Provide feedback to parents as needed. Enter documentation into the client's electronic health record as appropriate (i.e., incident reports, health care visits, immunizations, etc.) Participate in the assessment, treatment planning, and discharge planning of each child on the caseload as needed by communicating with DCBS workers, clients, and families and attending family court and any other appointments relevant to the child's care.

Family Consultant / Foster Care Case Manager Skills and Qualifications

Bachelor's degree in psychology, social work, education, counseling or related human service field Valid driver's license, insurable driving record and reliable transportation required Direct experience working with youth who experience emotional and/or behavioral disturbances in a treatment or education setting is highly preferred Experience working with learning-based treatment plans or behaviorally oriented programs is highly preferred Ability to travel as needed within the community, foster homes, and other places related to on-call, recruitment, and home certification across both program regions
