to join our team. This position is responsible for assisting clients with all case management concerns/issues as they relate to treatment. The focus is on the planning, implementation, and/or coordination of all treatment services delivered to clients that are assigned to primary counselors. This includes ensuring that established procedures are followed and that clinical standards are set for the delivery of individual case management services, and discharge planning and referral services are met. Collaboration and coordination with both internal and external resources are crucial to successful client outcomes.
Duties
Responsible for service coordination which establishes a framework of action utilizing internal and external resources to provide effective case management.
Collaboration with the client and significant others, coordination of referral services, liaison activities with community and family and managed care systems, and client advocacy.
Complete necessary administrative procedures at admission to treatment.
Complete case management intake within 48 hours of admission.
Contact the client's family/significant others to inform them of policies and procedures as they apply to family sessions, counselor assignment, and other relevant information.
Establish accurate treatment and recovery expectations with clients.
Development of an aftercare plan for each assigned client.
Successful collaboration with clients and significant others.
Responsible for the collaborative process that facilitates the client's progress toward mutually determined treatment goals that specifically pertain to case management services.
Establish a helping relationship with the client characterized by warmth, respect, genuineness, concreteness, and empathy.
Facilitate the client's engagement in the treatment process.
Promote the client's knowledge, skills, and attitudes that contribute to a positive change in substance use behaviors.
Encourage and reinforce client actions determined to be beneficial in progressing toward recovery.
Work appropriately with clients to recognize and discourage all behaviors inconsistent with progress toward treatment goals.
Adapt strategies to the individual characteristics of the client, including but not limited to disability, gender, sexual orientation, developmental level, culture, ethnicity, age, and health status;
Responsible for facilitating the client's use of available support systems and community and family resources to meet needs identified in clinical evaluation and/or treatment planning.
Additionally, responsible for ensuring that the treatment team is aware of the client's case management needs.
Apply confidentiality rules and regulations appropriately.
Demonstrate respect and non-judgmental attitudes toward clients in all contacts with community professionals and agencies.
Exchange relevant information with the agency or professional to whom the referral is being made in a manner consistent with confidentiality rules and regulations and generally accepted professional standards of care.
Size and quality of networks established.
Readiness of access to needed resources, quality of resources accessible, and demonstration of effective working relations;
Knowledge of and access to network database: efficiency and effectiveness of client referral;
Quality of working relations: timeliness and appropriateness of consultation, the ability to cooperate, and the results of collaborative efforts;
This position is also responsible for issuing reports and communications that are called for in the completion of assigned duties.
All notes and any other documentation are completed according to policy, are legible.
All reports and/or documents requested by management are accurate and timely.
Ensures efficient programming occurs through volunteering coverage as needed.
Anticipates the needs of the clients and the program in order to ensure effective programming.
Responsible for adhering to all applicable Federal and State laws, the professional code of ethics prescribed by the Division of Consumer Affairs Licensure Board of New Jersey; the policies and procedures detailed in the Policy and Procedure Manual, Code of Ethics statement and the instruction provided by supervision to include alternative duties.
Qualifications
Bachelor's degree in social work, psychology, or related field.
Organizational and administrative skills
Able to communicate effectively (both written and oral skills)
Ability to work independently
Ability to work under pressure
Familiarity with health and safety issues
Two years of Sobriety prior to application (if applicable).

Pay: $48,000.00 - $53,000.00 per year
