Qualifications:

The HHDS Case Manager is a source of support and guidance to individuals affected by intellectual disabilities and related conditions. They identify individuals' resources and needs, and then develop an Individual Plan to increase self-sufficiency within the individual.

The CM position requires the CM to be able to communicate effectively both verbally and in writing; the ability to work independently, cooperatively, reliably and dependably; the ability to establish and maintain effective relationships with individuals whom we provide contracted services with and their significant others; the ability adhere to, teach, and enforce HHDS, Rural Regional Center (RRC), Nevada State, and federal policies, procedures, and guidelines; the ability follow directions from HHDS upper management, Service Coordinators, their supervisors, and Quality Assurance staff from RRC; the ability to supervise/support direct support staff; the ability to assemble meaningful information in a logical order and draw valid conclusions from that information; and the ability to manage available resources to carry out assigned tasks.

Primary Responsibilities:

Adhere to all required company, state and federal policies, procedures and guidelines. Document and bill for in case notes all allowable communication/interactions, observations, and activities related to the individuals receiving services from HHDS. Maximize monthly billing according to assigned authorized contracted CM hours. Face to face and phone contact with the individual and his/her direct support staff to ensure the individual's health, wellness, and safety needs are being met. Ensure all individuals receiving services from HHDS experience the highest quality of life available through supports/services contracted for by RRC. Ensure all necessary and/or required paperwork for individuals on your caseload is completed, including Medicaid, SSI/SSA, SNAP, etc., applications and re-determinations. Ensure staffing issues and concerns are addressed. Interview/ hire, supervise, train, discipline (when necessary) and provide written evaluations on the direct service staff under your supervision. Review and approve direct service logs/documentation utilized for monthly billing. Review and approve direct service staff schedules. Schedule and facilitate monthly staff meetings during which time the CM also provides required staff training. Liaison with people, agencies and organizations involved in the individual's life. Assist with scheduling appointments for the individual, including medical appointments. Act as an individual's "representative payee" with Social Security when required. Complete written quarterly reports for each individual on your caseload. Attend Annual PCP, quarterly and other meetings (as needed) for individuals on your caseload. Create/update individual "Hab Plans" based on the individual's current PCP Support Plan. Communicate effectively in verbal and written forms with individuals served, their families and/or significant others, coworkers, supervisors, other service agencies, and the community. Respond appropriately to crisis and emergency situations, including providing direction to staff, reporting to and/or responding appropriately your supervisor and/or to other relevant state/federal agencies. Accurately complete all other duties as assigned by your supervisor and/or management. Adhere to and comply with all HHDS and RRC policies, procedures and training's. Other duties as assigned.
Applicant must be 18 years or older, have high school diploma or GED, able to pass background check & random drug tests, if a Valid Nevada driver's license, must have clean driving record.
