Job Summary

We are seeking a

dedicated and compassionate Social Worker

to provide

Community-Based Rehabilitation Services (CBRS)

and

Case Management

for individuals with mental health needs.

You'll help clients build skills, access resources, and work toward recovery and independence--all while collaborating with a caring, multidisciplinary team.

Responsibilities

Provide

individual CBRS

and

case management

services Deliver

crisis intervention

and support Use

behavioral therapy techniques

to address client-specific goals Coordinate care with a multidisciplinary team Maintain accurate and timely documentation (including ICD-10 coding)

Qualifications

Eligible to work as a

Case Manager

and

CBRS provider

under

State of Idaho

rules Experience working with individuals experiencing

severe mental health issues

and

crisis situations

Strong knowledge of

behavioral therapy

and recovery-based practices Excellent

communication

and

interpersonal

skills Proficient in

Microsoft Word

and

Excel

Valid Idaho driver's license

and

reliable transportation

required
