Case Manager at Adult Day Care Center



Position: Case Manager Location: Bronx NY Type: Full-Time



: We are seeking a dedicated and compassionate Case Manager to join our team at our Adult Day Care Center. The Case Manager will play a pivotal role in enhancing our existing services by conducting thorough assessments of clients' long-term goals during intake and developing individualized service plans. These plans will incorporate small, achievable tasks aimed at fostering productive daily experiences for our clients.



Principal Responsibilities:


Manage a caseload of clients.

Complete Assessment Summaries and determine the type of services needed for each client.

Develop individualized service plans focusing on recovery and wellness.

Maintain detailed service documentation, including client progress notes, observations, interactions, and incident reports as necessary.

Provide crisis intervention support.

Participate in individual service coordination activities, including services, workshops, and support for recreational and socialization activities.

Maintain the program's resource directory.

Refer clients to appropriate community providers to address their needs.

Perform all other duties as assigned.


Minimum Qualifications:


Bachelor's degree.

At least 1 year of experience working with the elderly and/or adult mental health community.

Ability to develop and implement treatment plans.

Strong problem-solving and crisis intervention skills.

Spanish speaking required.
