Duties
Conduct thorough assessments of clients' needs, including behavioral health, addiction counseling, child welfare, and workforce development.
Develop individualized care plans tailored to each client's circumstances and goals.
Provide crisis intervention and crisis management to stabilize clients during emergencies or high-stress situations.
Facilitate access to services such as Section 8 housing, Medicare, Medicaid, and other community resources.
Offer motivational interviewing and mentoring to encourage positive behavioral change.
Supervise and coordinate with service providers, including childcare providers and social service agencies.
Monitor client progress through regular follow-ups and adjust care plans as needed.
Document all interactions accurately in compliance with organizational policies and regulatory standards.
Advocate for clients' rights and assist them in navigating complex systems such as child welfare or workforce programs.
Qualifications
Proven experience in social work, behavioral health, or related fields with supervising experience preferred.
Strong skills in crisis intervention, crisis management, and addiction counseling.
Knowledge of community resources including Section 8 housing, Medicare, Medicaid, and workforce development programs.
Excellent communication skills with the ability to motivate and mentor clients effectively.
Ability to handle sensitive situations with professionalism and compassion.
Relevant certifications or licenses related to social work or counseling are desirable but not mandatory. This position requires a dedicated individual committed to supporting vulnerable populations through compassionate care and resource coordination.
