Job Summary and Qualifications

Serves as a resource to staff and as a support to the director. Facilitates communication between Care Coordination staff, clinical departments, PTAC, PAS and HIM.



Position Requirements



Required Registration, Licensure or Certification

Current Licensure in the State of Kansas as a Registered Nurse (RN) or Current Multi-State Licensure as a Registered Nurse or Licensed Social Worker Current Basic Life Support (BLS) verification course through American Heart Assoc. or American Red Cross - Healthcare Provider CPR


Preferred Registration, Licensure or Certification

None


Required Education or Experience

Bachelor's degree 2 years of previous experience


Preferred Education or Experience

Bachelors of science in Nursing from an accredited institution Three years of clinical experience in acute care or case management


Required Knowledge and Skills

Demonstrates ability to communicate effectively Knowledge of accepted program planning and adult education standards and practice Knowledge of accepted Medical Social Services standards of practice Knowledge of laws and regulations affecting the organization Demonstrates excellence in customer service
