The Case Management Assistant serves as the central point of contact for Case Management Staff, vendors, and patient families. The Case Management Assistance receives and triages phone calls on a multi-line phone system and receives walk-ins.
Qualifications
Education:
High school diploma required.
Associate degree in social work, family service, or closely related field preferred.
Experience
Two years' prior experience working in an office environment required.
Prior experience working in healthcare environment preferred.
Previous social work experience preferred.
Licensure/Certification/Registration
None
Duties & Responsibilities: Retrieve signatures on required paperwork, such as Medicare Forms, Observation Forms, and Medicare important Message Forms.
Assists patient family members by communicating referral information, inpatient follow-up, and case manager information.
Responsible for various related projects that may change over time but include computer data input, payroll tracking, supply inventory and ordering, scheduling meetings, and resource scheduling.
Provide information for discharge planning, maintaining meeting minutes, and monthly reports.
Provide exemplary core customer service.
Work effectively and collaboratively with colleagues, physicians, and department heads.
Effectively utilize strong organizational skills.
Consistently display effective verbal and written communication skills.
Proficient understanding and use of technology/PC skills required.
