Duties



Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts or resident's lifts. Monitor patients' vital signs and observe for any changes in health status, promptly reporting concerns to medical professionals or supervisors. Prepare nutritious meals tailored to individual dietary needs, ensuring proper food handling and cleanliness standards are maintained. Administer medications accurately following care plans and HIPAA regulations to protect patient privacy. Support clients with memory care needs related to Alzheimer's disease or dementia by implementing specialized routines and safety measures. Maintain detailed documentation of care provided, patient observations, medication administration, and progress notes in compliance with healthcare regulations. Engage clients in social activities and provide emotional support to foster a positive environment that enhances mental well-being. Perform light cleaning duties within the client's living space to promote hygiene and comfort. Follow all safety protocols during patient handling, including heavy lifting techniques and proper use of assistive devices like Hoyer lifts or resident lifts. Collaborate with healthcare teams, social workers, and family members to develop and update personalized care plans aligned with each individual's needs. Adhere strictly to HIPAA guidelines to ensure confidentiality of all patient information at all times. Participate in ongoing training related to CPR, first aid, hospice care, autism support, working with people with disabilities or developmental disabilities, and other relevant certifications.

Experience



Prior experience working in assisted living facilities, nursing homes, home health agencies, or group homes is highly preferred. Demonstrated expertise in patient monitoring, medication administration, and long-term care practices. Familiarity with specialized care such as Alzheimer's/dementia care or hospice environments is advantageous. Experience working with individuals with developmental disabilities or autism enhances your ability to provide tailored support. Knowledge of safety procedures involving heavy lifting and the use of mobility aids like Hoyer lifts is essential. Strong communication skills for effective collaboration with healthcare teams and family members. Proven ability to follow detailed care plans while maintaining flexibility for changing needs. Certifications such as CPR, first aid, HIPAA compliance training, or nursing assistant credentials are a plus but not mandatory for all roles.
