Position Summary:




The Care Partner is a key member of our team, providing compassionate, person-centered care in a warm household environment. This role supports residents with daily living activities, personal care, meals, housekeeping, and laundry while following individualized plans of care. Care Partners build meaningful relationships, encourage resident independence and participation, and help create a safe, clean, and welcoming home. Working collaboratively with a supportive team, the Care Partner communicates resident needs, maintains accurate documentation, and upholds safety and infection control standards. Key Responsibilities:



Knowledgeable about each household resident's Plan of care. Provide meal preparation under the direction of the Director of Dining. Clean resident room and common areas. This includes dusting, vacuuming, floor cleaning, cleaning bathrooms, keeping kitchen tidy and clean. Provides recreational activities to residents, according to residents' preference and under the supervision of the Household Guide. Work well with household and neighborhood teams to maximize resident potential, comfort, and happiness. Participate in care partner scheduling, being flexible, and willing to cover shifts as necessary. Perform residents' personal laundry. Encourage residents' involvement in household life, encouraging them to participate in household tasks to the extent their condition permits, and the resident is interested. When working 11-7 shifts, maintain accurate census record. Follow procedures to provide resident care: bathing, oral hygiene, transferring to/from bathroom, use of commode, keep incontinent residents always clean and dry, change bed linens as necessary. Transfer, ambulate and position residents to ensure comfort, prevent injury, and to promote optimal levels of function. See that residents are properly clothed at all times, i.e. clothing not soiled. Follow standard procedures to provide personal grooming: trims nails, combs hair, shaves male residents daily and shampoos hair as indicated and informs supervisor if changes occur. Make beds, keep nightstands, dressers and closets in orderly fashion. Assist resident at meals, serves, distributes beverages as needed, assist residents with cutting foods as needed, provides nourishment when indicated, encourages and monitors meal acceptance. Prepare residents for tests, X-rays, visits from physicians/dentists/therapists as directed by Supervisor. Encourage residents to participate in recreational activities, church, facility events. Supports the residents' full participation and assists in transporting them to/from various activities. Perform personal services for residents such as writing/reading letters, using TV and radio. Promotes comfort, security, independence, and resident safety. Assist and communicate effectively with relatives, visitors, and guests. Direct inquiries and comments regarding resident health to nurse in charge. Maintain appropriate techniques in all resident care to prevent the spread of disease and infection. Observe residents and informs nurse of any change in attitude, reactions, appetite and/or other significant physical changes as they occur. Answer call lights and call bells to determine resident needs. Answer telephone calls and immediately relay messages to the proper source. Follow plan of action to maintain safety of the residents and others in the event of fire or disaster. Assist the Neighborhood Director with orientation of Care Partner. Maintain accurate records on resident activities and functions such as TARS, Toileting Schedule, etc. Maintain a clean, safe, and orderly work area. Reports safety hazards. Complete 12 hours in-service training. Always adhere to dress code policy. Always adhere to time and attendance policy. Practices safe work habits and follows all safety procedures of the facility. Assures the safety, security, and wellbeing of our residents. Minimum of one year of work experience with the elderly, memory care preferred Must have High School diploma or two years of related work experience. Basic first aid and CPR.

Key Requirements Other:



Must be able to read, write, speak, and understand the English language. Must possess the ability to make independent decisions when circumstances warrant such action. Must possess the ability to deal tactfully with staff, residents, family members visitors, government agencies/staff, and the general public. Must be knowledgeable of practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care. Must possess the ability to plan, organize, develop, implement, and interpret department programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care. Must have patience, tact a cheerful disposition and enthusiasm, as well as the willingness to handle challenging residents/associates. Must be able to communicate with the staff, and other department supervisors. Maintains a rapport with the residents and their families. Must be willing to seek out new methods and be willing to incorporate them into existing practices. Must not pose a direct threat to the health or safety of other individuals in the workplace. Must attend and participate in continuing educational programs.

Physical and Sensory Requirements:



Must be able to move intermittently throughout the work day. Must be able to cope with the mental and emotional stress of the position. Must meet the general health requirements set fourth by the policies of this facility. May be necessary to assist in the evacuation of residents during emergency situations. Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met. Must be able to push, pull, move, and/or lift a minimum of fifty (50) pounds. Must be able to climb stairs.
