Summary of Responsibilities



You would be responsible for overseeing and coordinating the care of Aging Wisdom clients and their families. As a Care Manager, you are often the primary advocate for your clients. This position involves:




Conducting home and facility visits in the Greater Seattle area Performing biopsychosocial assessments; developing, implementing, and monitoring care plans Collaborating with providers across the healthcare continuum Supporting clients in crisis and during end of life Navigating complex family dynamics and other challenges Participate in community professional networking and business development opportunities Initiate relevant learning and professional growth opportunities Maintaining familiarity and knowledge of local community resources Collaborating with supervisor and larger care management team in regular meetings Providing holiday rotational on-call coverage Adhere to professional code of ethics and standards of practice Be committed to excellence at every level




In addition to our work seeing clients in the community, we also work remotely from our homes or our Queen Anne office. Flexibility, mentoring, team collaboration, ongoing training, and our close-knit team culture make this a unique opportunity.

Education, Experience, and Skills Required




Master of Social Work or similar graduate degree At least 1-2 years of experience working with older adults (examples include home health, hospice, in-patient hospital experience, skilled nursing, home care, senior centers, adult day health, medical facilities, and/or senior living communities)



Compensation/Benefits



Salary $75,000-$90,000 (based on related experience) PTO Health, Vision & Dental Insurance 401k retirement plan with 3-5% match 11 paid holidays Ongoing education and training; professional development opportunities Clinical supervision for those seeking licensure Membership dues for The Aging Life Care Association Assistance in obtaining Care Management Certification
