Your duties will include:
Providing support/education to clients with complicated social/behavioral/environmental issues
Conducting home visits
Utilizing clinical judgment and critical thinking skills to facilitate appropriate client physical and behavioral healthcare and coordinating psychosocial and medical wraparound services to promote effective utilization of available resources
Reporting specific health and social information back to the integrated team to assist in development of care/service plan
Advanced care planning with client and others involved with client care
Authorizing services in the most efficient means possible to meet identified client needs
Qualifications:
Ohio LSW or RN licensure
At least one year of experience working with persons with disabilities/chronic conditions and LTSS.
Prior or current experience with mental/behavioral health populations
Prior experience completing electronic documentation
Prior case management experience and experience working in a program with strict and multiple contractual requirements is a plus
Critical thinking skills
Ability to effectively use clinical judgment
Ability to learn in-person, through a written format and virtually
Have above average technology skills
Ability to work in multiple software systems simultaneously
A good working knowledge and understanding of issues related to the elderly population.
Knowledge of community resources within Southwestern Ohio.
Good oral and written communication skills
A valid driver's license and reliable transportation for travel and conducting home visits and attending trainings
Proficient computer skills including the Windows operating systems, MS Word, MS Excel, data entry and the ability to learn new software applications as needed
