Summary:
The School Based Behavioral Health Patient Care Coordinator is an integral member of the patients care team. The BH Patient Care Coordinator works collaboratively with other primary care providers to ensure that the behavioral health provided meets the patients needs. The Patient Care Team involves the patient\xe2\x80\x99s primary care provider, nurse, medical assistants, and/or mental health providers in the health center or school-based office, in addition to other outside agencies or providers as appropriate. The BH Patient Care Coordinator is the liaison between the patient and other health care team members and is responsible to ensure that the treatment plan put in place by the care team members is working effectively for all involved.
The School Based BH Patient Care Coordinator will provide excellent customer service to patients and providers, while ensuring access to care, assessment for additional services if qualified, and coordination of non-traditional healthcare services to ensure whole patient care is achieved.
While the School Based BH Patient Care Coordinator\xe2\x80\x99s primary role is as a liaison between the patient and the interdisciplinary care team they have some key responsibilities which include:
Creating behavioral health goals for the patient, developing a treatment plan with other care team members, assessing and screening patients, helping patients reach out to community resources and coordinating the care with the interdisciplinary care team members insuring everyone is following the same care plan and the patient is receiving the best possible care.
The School Based Behavioral Health Patient Care Coordinator is an active participant in clinical team huddles and care planning activities. In addition, the SBHC BH Patient Care Coordinator participates in education and outreach events as needed in the community. See essential responsibilities below.
