Summary:



The School Based Behavioral Health Patient Care Coordinator is an integral member of the patients care team. The BH Patient Care Coordinator works collaboratively with other primary care providers to ensure that the behavioral health provided meets the patients needs. The Patient Care Team involves the patient\xe2\x80\x99s primary care provider, nurse, medical assistants, and/or mental health providers in the health center or school-based office, in addition to other outside agencies or providers as appropriate. The BH Patient Care Coordinator is the liaison between the patient and other health care team members and is responsible to ensure that the treatment plan put in place by the care team members is working effectively for all involved.


The School Based BH Patient Care Coordinator will provide excellent customer service to patients and providers, while ensuring access to care, assessment for additional services if qualified, and coordination of non-traditional healthcare services to ensure whole patient care is achieved.


While the School Based BH Patient Care Coordinator\xe2\x80\x99s primary role is as a liaison between the patient and the interdisciplinary care team they have some key responsibilities which include:
Creating behavioral health goals for the patient, developing a treatment plan with other care team members, assessing and screening patients, helping patients reach out to community resources and coordinating the care with the interdisciplinary care team members insuring everyone is following the same care plan and the patient is receiving the best possible care.


The School Based Behavioral Health Patient Care Coordinator is an active participant in clinical team huddles and care planning activities. In addition, the SBHC BH Patient Care Coordinator participates in education and outreach events as needed in the community. See essential responsibilities below.

  • Support and closely coordinate mental health care with the patient\xe2\x80\x99s primary care provider, mental health providers, specialists, or other community resources;
  • Contact and follow-up with parents and guardians to screen and gather information and intake paperwork from patients, ensuring they are signed and renewed;
  • Update and maintain client spreadsheets;
  • Travel between schools, pick up and drop off intake packets, provide resources and supplies to our School Based Counselors, including copies of intake packets;
  • Attend and participate in meetings, back to school events, parent teacher conferences and other events and activities as required, including set-up for Back-to-School events;
  • Conduct PREPARE, legal and additional documents needed for current clients;
  • Use evidence-based tools to track patients (in person and by telephone) for changes in clinical symptoms and gather information about treatment side effects or complications; coordinates with other members of the care team as needed;
  • Facilitate in-clinic referrals for other Alluvion Health services such as outreach and enrollment, dental, prescription assistance, etc.;
  • Participate in regularly scheduled caseload consultation with the care team;
  • Support patient engagement and follow-up in care;
  • Evaluate patient care plans with care team;
  • Document patient encounters, patient progress, treatment, and follow-up care in the electronic medical record system within the scope of practice;
  • Facilitates referrals for clinically indicated services outside the community health center (mental health specialty care, substance abuse treatment, housing assistance, etc.);
  • Attend and participate in meetings and Quality Improvement activities as required;
  • Serve as a member of committees as requested;
  • Participate in peer review as part of Quality Improvement activities and in compliance with health center policies;
  • Maintains confidentiality according to HIPAA;
  • Works as a team player with staff and other community organizations to ensure quality services and program requirements are met to carry out the goals and objectives of the health center program;
  • Performs duties efficiently and effectively;
  • Follows Alluvion Health policies and procedures;
  • Establishes and maintains productive working relationships with fellow employees, supervisors, and the public;
  • Performs other duties or projects as required to accomplish the objectives of the position;
  • Ensures compliance with HIPAA/HITECH;
  • Maintains compliance standards in accordance with the HRSA compliance policies, HIPAA, HITECH, FTCA and the Code-of-Conduct and reports concerns timely and appropriately;
  • Performs a variety of other duties as assigned; which may include but are not limited to: directing or participating in special projects and events, conducting research, representing Alluvion Health at meetings and conferences, and attending continuing education and training events.

Education/Experience/Qualifications:


Education/Training:


  • Three (3) to five (5) years related experience required;
  • Associates or Bachelor\xe2\x80\x99s Degree preferred.
  • Experience in a Federally Qualified Health Center or Healthcare Setting preferred.



Certifications:


  • Behavioral Health Aide, Mental Health Aide, Medical Assistant, Chronic Care professional, LPN, or RN preferred.
  • Certified Application Counselor and/or SHIP certification experience preferred
  • Must possess a valid driver\xe2\x80\x99s license issued by the State of Montana.



Language Skills:



Fluent in the English language. Ability to read, analyze, and interpret the most complex documents. Ability to respond effectively to the most sensitive inquiries or complaints.

  • Mobility that permits the employee to move about in a variety of building settings; Personal mobility that permits the employee to enter, operate and exit motor vehicles and travel to other clinic sites.



Working Conditions:



  • Work indoors in climate-controlled environment 95% of the time.

  • OSHA Exposure Category #2 (The normal work routine involves no exposure to blood, body fluids, or tissues, but exposure or potential exposure may be required as a condition of employment.)



Work Hours:



  • Full-time, non-exempt; typically a 40-hour work week. Some evenings and weekend hours as needed.
