Hybrid work-from-home opportunities are available after the first 6 months of employment! JOB SUMMARY: As a Care Coordinator, you will help members of Minnesota Health Care Programs make informed choices about their health care services, conduct assessments, coordinate services, and help members make progress toward their health goals. In addition, you will be responsible for the process of ongoing assessment, planning, referral, orientation, monitoring, and advocacy assistance through which multiple medical service needs of members are addressed. REQUIREMENTS: Bachelor\'s degree in Social Work, Psychology, Sociology or closely related field AND maintain licensure with MN Board of Social Work once license is obtained OR a bachelor\'s degree in any field AND one year of experience as a social worker in a public or private social service agency. A closely-related field includes occupational therapy, physical therapy, speech-language pathology, audiology, recreational therapy, dietician, special education, rehabilitation counseling, nursing, human services or other field in health or human services that involves the job duties of a care coordinator/case manager, including assessment and service planning. Experience with Medical Assistance, Medicaid, Elderly Waiver, and Health Plan programs preferred Candidates should have expert level knowledge of community resources for the aging/disabled population Willingness to work with diverse populations Willingness to learn and work in multiple electronic record keeping systems simultaneously Bilingual candidates preferred Must be at least 18 years of age Background clearance Ability to effectively communicate in English, written and verbal Driver\'s license, acceptable driving record, and proof of auto insurance required To view other open positions, click here.
