We are currently looking for a Full Time Care Coordination Nurse Educator to join our NLD Inpatient Team located at the Chickasaw Nation Medical Center working 8:00AM-5:00PM - Mo Tu We Th Fr.


Summary:





Responsibilities:



Promotes the mission, vision, and values of the Chickasaw Nation Department of Health Demonstrates effective, professional communication and collaboration with the Unit Leadership, Unit Staff and Nursing Learning and Development Provides and advocates age and culturally appropriate, holistic patient care based upon facility mission statements and as outlined in policies and procedures Ensures that staff competency levels are assessed on a regular basis and that they meet a minimum standard across the inpatient setting Acts as liaison between contract agencies and hospital leadership to ensure compliance with credentialing, orientation, and performance standards Collaborates with unit managers, unit educators, and quality department on educational needs of care coordination staff Collaborates with units across the inpatient setting on educational needs of care coordination staff Stays abreast of Electronic Health Record and Pyxis processes and acts as super-user to staff as needed Initiates and completes all aspects of educational programs in a timely manner and maintains personnel educational data in an organized fashion Works closely with unit managers and Nursing Learning and Development staff to coordinate and develop education opportunities for unit staff Participates in hospital and nursing education programs and ensures that patients and staff members' educational needs are met Verifies that educational resources are available and implemented across the inpatient setting Assists with hospital and departmental performance improvement activities to measure, assess, and improve the quality of care delivered to patients Plans, prepares, and maintains available educational materials/resources that are accurate, relevant, and evidence based Provides guidance, support, and constructive feedback to staff as indicated Directs staff in the orientation of new employees in the facility and in other staff developmental programs Has the operational responsibility and accountability for meeting educational needs by incorporating advanced clinical practice, education, consultation, research, and professional/leadership activities in all phases of education including assessing, planning, implementing, communicating, and evaluating educational programs Ensures documentation and maintenance of initial and ongoing competency of all levels of staff within the inpatient setting including appropriate competencies for agency and contract staff that work in the inpatient setting Participates with members of medical staff and administrative staff in planning patient care standards, protocols, policies and procedures Maintains confidentiality, security, and integrity of data and information Assists Care Coordination staff in evaluating practice and creating strategies for further improvement Provides education on changes within the inpatient setting and the Chickasaw Nation Department of Health Pursues professional growth and participates in professional organizations to stay updated on the latest best practices for inpatient nursing Provides support and assists with educational programs Adjusts content and teaching strategies appropriate to leaner needs and provides educational opportunities that benefit nurses at all stages in their career development Develops appropriate education and training on new equipment across the inpatient setting Works closely with Nurse Educators and unit leadership to coordinate and develop educational opportunities for the Care Coordination staff Uses high-fidelity simulation and debriefing for staff training Oversees compliance and assists staff with HeartCode and RQI programs and assignments Attends and actively participates in departmental meetings Attends organizational meetings at the discretion of the unit manager Ensures effective communication with employees regarding new policies and procedures, guidelines, protocol, performance improvement activities, customer satisfaction results, administrative memo/updates, and hospital initiatives Participates in emergency preparedness processes and activities to ensure staff readiness Assists as needed with the development, organization, and coordination of the nursing department onboarding program and makes changes as necessary Writes competency statements and related performance criteria Develops annual competency-based education with the discretion of the unit manager and nursing learning and development Participates in annual competency validation Supervises the identification of needed core and unit clinical competencies and performs competency evaluation/skills assessments Provides first class customer service - courteous and helpful to all internal and external customer types Answers questions in a courteous and professional manner Greets and engages with a smile Determines needs, seeks positive experience and outcomes Completes all training and development requirements and participates in continuing education opportunities Works in a collaborative and supportive manner to support the productivity and safety of the work area, team members and customers of all types Maintains general upkeep of work area and maintains a safe environment Maintains and promotes professionalism Punctual and consistent in meeting start time, work schedule, attendance and other assigned work time metrics Adheres to Chickasaw Nation dress code standards Maintains knowledge of and follows all laws, rules, regulations, policies and SOPs for assigned area, e.g. MICS, TICS, hospital standards, health codes, food sanitation, accounting principles, etc. Performs all other duties as assigned
