Three to five years of administrative support services required, medical office experience preferred. Bachelor's degree in health, human services or related field preferred. Ability to communicate effectively with others to establish professional relationships and ensure client satisfaction. Excellent organizational skills with the ability to manage multiple priorities and the flexibility to adapt to changes that occur within an evolving position. Proficient in Microsoft Office Suite (i.e., Access, Excel, Word, PowerPoint, Outlook) and general office equipment (i.e., Computer, Printer, Copier, Scanner, Facsimile, Calculator, Postage Meter).




What You Will Do:




The Care Coordination Associate (CCA) is responsible for supporting the Medical Day Care Waiver (MDC) team by coordinating and administering specific activities for individuals served by MDC and the MDC coworkers. This position provides intake, referral and clerical support and serves as the primary contact for the internal client record that is maintained in The Center's documentation system. This position requires a customer service perspective to internal and external customers. This role also requires CCA's to adjust their workload and develop specific goals and plans to prioritize, organize and manage the multiple priorities of the MDC program. CCAs are mainly remote, however, they must be able to go into the office to collect mail and other documents needed for the team. They must also be able to provide a consistent daily presence for prompt response to client, family, case manager, and provider needs/inquiries. This position reports to the Program Director but works jointly as a member of the MDC team to maximize quality outcomes for individuals served by The Center.



As a Care Coordination Associate, you will:




Intake


Monitor LTSS for new referrals and enter the demographic information in the internal case management information system (CMIS). Conduct initial contact with client/family to verify demographic information, introduce The Center, explain the program, and enrollment process. Provide initial consent documents to client either by email (using Jotforms) or verbally. Conduct follow-up to assist Case Managers and clients throughout application process to program enrollment. Review and address all alerts received by MDH pertaining to client assignment, deactivation, eligibility, and address changes; Communicate with Case Managers and Team manager about deactivation and transfers.
Maintain an accurate list of participants- those who are actively receiving services, those who are awaiting services, and those who have been discharged from services; Submit Authorization to Participate (ATPs) to assist with enrollment changes.

Customer Service


Triage calls and other inquiries from clients/families, providers, and outside agencies; Address any issue/concern or providing appropriate follow-up with assigned case manager to address issue/concern. Locate and organize available resources and resource lists. Provides general information and handles general questions about care coordination and/or the MDC program. Complete other duties as assigned by Case Managers, Team Managers or Program Director (i.e., process applications and forms, mail outgoing correspondence/process incoming correspondence, create forms, special projects/tasks, etc.). Demonstrates the ability to respond to changing situations in a flexible manner to meet current needs, reprioritizing work as necessary.

Care Management - Administrative


Provide primary clerical support including written correspondence (reports, letters, consents) as well as a variety of other duties including scanning, e-faxing, organizing/processing documents and filing. Perform administrative support tasks such as recording program meeting minutes. Serve as Administrator for councils/committees (perform administrative tasks such as taking meeting minutes, scheduling meetings and assisting facilitator with other duties as needed). Maintain electronic client files by scanning, uploading, and filing. Demonstrate knowledge and understanding of all administrative general office policies and procedures and the ability to reference them from appropriate books, manuals or computer sites. Prepare legally required and special reports.
