Overview




Details: Monday through Friday from 8:00am until 4:30pm. Supervisor will be on call every 4 weeks, plus on call rotation for holidays. Supervisor will be responsible to provide coverage for team members when one is not found, if afterhours and weekends.


Remote Training - in office for the duration - this will depend on the supervisor's comfortability with the position.


Understanding of homecare or arranging referrals is a must have!


Assists the Director in the overall operation of Referral Services statewide. Accountable for day-to-day operations through implementation of strategy, policy, direction and action plans. Ensures the overall efficiency and effectiveness of services and functions associated with post-acute care transitions under the broad umbrella of Ambulatory Services and New Business Integration (AS&NBI) Care Alliance Services. Provides direct supervision in the efficient operations of Care Alliance Referral Services to home care agencies or extended care facilities. Supervises intake/referrals, insurance verifications, documentation, and customer service functions. Ensures all functions are performed properly and efficiently. Trains and directs staff, and serves as a liaison with external customers such as patients, physicians, hospital departments and external care agencies. Responsible for ongoing education and compliance requirements for team. Leads team in reviewing timely discharges to ensure patient safety. Collaborates with Throughput Services and statewide care agencies.



Bachelor's Degree or equivalent years of experience is required.

Requires 5-7 years of related experience. Requires strong verbal and written communication skills. Requires advanced working knowledge of Microsoft Excel, Word and PowerPoint software applications. Must have excellent organizational and project management skills and ability. Requires strong analytical skills and is an accomplished problem-solver.
