New Horizon Family Health Services (NHFHS) is a Federally Qualified Health Center (FQHC) that has been improving the health of the communities we serve since 1992. Details:

  • Under the direct supervision of the Care Management Supervisor, the Care Advocate will provide needs self-assessments, client/community advocacy information, health information, and referral to the health center. The Care Advocate will work with patients requiring assistance to obtain medications by helping with the forms required by pharmaceutical companies. This position will provide patient assistance with accessing other health and social support resources.
All NHFHS Company employees must be vaccinated against COVID-19.

Experience and Skills:

Education:
  • Bachelor\'s in Social Work, LBSW preferred
  • BS in Psychology, Human Services or related field
Knowledge and Skills:
  • Must have three years in a clinical setting
  • Must demonstrate legible handwriting, excellent spelling, being a motivated and enthusiastic learner with a courteous and tactful manner
  • Ability to health clients understand and prioritize individual and family needs
  • Have a working knowledge of the community health care system and the ability to understand Medicaid eligibility requirements with the certification/recertification process.
  • Have a general understanding of illness, disease processes, and medical condition as they relate to the behavioral and emotional wellbeing of all patients
  • Must have a reliable mode of transportation and possess a valid South Carolina driver\'s license.
