Bilingual Supervisory Nurse (RN/LPN) - Pediatric In-Home Care (GAPP Services)

PRN | Independent Contractor (1099) | Georgia Pediatric Program (GAPP)

Pay:

$150 PER INTAKE/RENEWAL $75 PER SUPERVISORY VISIT OTHER DUTIES PAID PER SERVICE

What You'll Do

Conduct

intake assessments, renewals, and supervisory visits

per GAPP and agency standards Perform

quarterly in-person supervisory visits

as required by GAPP guidelines Act as the

member's case manager

, ensuring individualized care plans are accurate and up to date Complete client assessments and develop or update

Plans of Treatment (PPOTs)

Supervise PCAs every 2 weeks

and

LPNs every 4 weeks (quarterly in-person)

Review documentation, timesheets, and care notes for accuracy and compliance Train, orient, and evaluate care staff to ensure competence and quality care delivery Communicate effectively with families, caregivers, and healthcare providers Maintain accurate, HIPAA-compliant records through

CareBravo (formerly Caryfy)

Report incidents, hospitalizations, or significant client changes promptly

What We're Looking For

Current

RN or LPN license

in the state of Georgia (in good standing)

Bilingual (English/Spanish)

- required Experience with

medically fragile pediatric clients

preferred Strong understanding of GAPP services and documentation requirements Excellent clinical judgment, time management, and communication skills Compassionate, reliable, and able to work independently
