We're looking for a Case Manager to join our team -- someone passionate about helping others, detail-oriented, and able to manage a dynamic caseload while building meaningful relationships with clients and caregivers.

Responsibilties:

Manage a caseload of

50-60 clients

, ensuring all care needs are met promptly and effectively. Match, coordinate, and train caregivers according to each client's care plan and preferences. Cover open cases as needed Maintain compliance by completing all required care assessments, documentation, and reports. Conduct

24-hour check-in calls

following the start of services. Conduct

10-day courtesy calls

and maintain regular communication with clients and caregivers. Complete

30-, 60-, and 90-day follow-up reports

for all clients. Communicate with

State Care Managers

and client families as needed. Ensure care plans are updated

annually

and reflect current client needs. Perform

bi-monthly home visits

to monitor satisfaction and care quality (

mileage stipend offered

). Notify your supervisor of client birthdays within your caseload to coordinate gifts. Participate in

on-call rotation

and provide support for

open cases

when necessary.
Qualifications:

Minimum

1 year of experience

in case management, home care, or a related healthcare field. Strong communication, organizational, and problem-solving skills. Ability to manage multiple priorities with compassion and professionalism. Valid driver's license and reliable transportation (required for home visits). Familiarity with state regulations and home care documentation preferred.
