Bereavement Coordinator - Hospice
Provides psychosocial/emotional assessment and intervention for bereaved individuals and families, primarily to family/loved ones of Halifax Health Hospice (HH-H) patients but also to community clients including traumatic loss clients. Coordinates and provides bereavement support and counseling in individual and group formats based upon assessment of needs. Maintains documentation per agency protocol. Discharges clients in a timely manner, refers to community professionals and agencies when needs of the bereaved go beyond scope of bereavement services. Assists in recruitment and training of volunteers. Participates in Bereavement staff meetings and other meetings as requested. Educates HH-H staff, volunteers and community on Bereavement services.

JOB QUALIFICATIONS:



Master's degree in social work, counseling, or related field. Board certified Music Therapist and Art Therapist for interventional counseling.


SKILLS, EXPERIENCE AND LICENSURE:



Social work or counseling background with experience in individual, family and group counseling. Knowledge of psychopathology, end of life care, grief and loss, and community resources. Strong assessment and clinical skills. Hospice experience preferred. Excellent communication skills and ability to work effectively with diverse populations of all ages. Ability to cope with high levels of stress and intense emotion. Valid driver's license with good driving record.


DUTIES AND RESPONSIBILITIES

:


Provides direct bereavement services to clients including traumatic loss clients in a timely and effective manner, including phone calls, face to face assessments, individual, family and group counseling. Enters appropriate logistical and clinical documentation in the electronic medical record. Serve as a consultant to the interdisciplinary team. Participates in the selection, training and retention of volunteers. Provides education to HH-H staff, volunteers, and the community. Makes referrals to community resources and professionals. Participates in community education and fundraising activities as requested. Meets requirements for continuing professional education for HH-H and State of Florida licensure. Performs other duties as assigned.


PHYSICAL DEMANDS:



Light to moderate physical effort, regular evening hours required to meet client schedules, occasional weekend hours.


WORKING CONDITIONS:



Primarily indoors in office setting, home and facility visits, community events/speaking engagements/fundraising activities may be indoor or outdoor in various settings.


SUPERVISION:



Works under direct supervision of Bereavement Manager. Supervision is ongoing in the form of direct observation, client satisfaction and annual performance evaluations.
