We are seeking a compassionate and dedicated Personal Care Assistant to provide essential support and care to individuals in various settings, including home, community, assisted living, and long-term care facilities. Duties
Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility
Monitor patients' vital signs including blood pressure, pulse, temperature, and respiration rates to ensure health stability
Support medication administration following prescribed care plans while maintaining compliance with HIPAA regulations
Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
Provide companionship and emotional support to promote mental well-being and reduce feelings of isolation
Observe and document patient conditions, behaviors, and any changes in health status for accurate reporting to healthcare professionals
Utilize specialized equipment such as Hoyer lifts or resident's to safely transfer individuals with mobility challenges
Maintain a clean and safe environment by performing light cleaning duties and adhering to infection control protocols
Assist with transportation to medical appointments or community activities as needed
Follow all safety procedures including CPR and First Aid protocols in emergency situations
Collaborate with healthcare teams to develop and implement personalized care plans for each resident or client
Support residents with memory care needs related to Alzheimer's disease or dementia through specialized techniques
Engage in ongoing training related to hospice care, autism spectrum disorders, developmental disabilities, or other specialized areas of patient care
Skills
Proven experience in home & community care, assisted living, nursing home environments or similar settings
Strong knowledge of patient observation techniques and vital signs monitoring
Familiarity with HIPAA regulations ensuring confidentiality of patient information
Ability to perform heavy lifting safely when required for resident transfers or mobility assistance
Excellent communication skills for effective interaction with clients, families, and healthcare teams
Compassionate approach with a focus on dignity and respect for all individuals receiving care
Ability to follow detailed care plans accurately while adapting to changing needs
Basic cooking skills for meal preparation aligned with dietary restrictions or preferences
Experience working with people with developmental disabilities or autism spectrum disorders is highly valued
Knowledge of Alzheimer's care, dementia management, memory care strategies and hospice support techniques is preferred
Certification in CPR and First Aid is required; additional certifications such as group home experience or nursing assistant credentials are advantageous
Strong organizational skills for documentation, medication administration records (MAR), and shift reports
