As we continue to grow, we are seeking a highly organized, proactive, and peoplefocused Scheduler/Care Manager to join our leadership team.
Job Summary
The Scheduler/Care Manager is responsible for coordinating and maintaining the daily and longterm scheduling of caregivers with clients, ensuring continuity of care, and supporting both staff and families with professionalism and empathy. This role is central to the smooth operation of BCH and requires exceptional communication, problem-solving, and time-management skills.
Key Responsibilities
Scheduling & Coordination
Create, manage, and adjust caregiver schedules to meet client needs.
Ensure all shifts are filled promptly, including last minute callouts.
Match caregivers to clients based on skills, availability, and compatibility.
Maintain accurate scheduling records in the agency's scheduling software
.
Willingness and flexibility to step in and provide direct client care when a scheduled caregiver is unavailable, ensuring uninterrupted, high-quality service for our clients.
Care Management
Serve as the primary point of contact for clients and caregivers regarding scheduling and service updates.
Conduct followups with clients to ensure satisfaction and quality of care.
Support onboarding of new caregivers, including orientation scheduling and documentation coordination.
Assist with care plan updates and communication with field staff.
Administrative Duties
Maintain compliance with state regulations and agency policies.
Document all schedule changes, incidents, and communications accurately.
Collaborate with the Care Director on staffing needs, performance concerns, and service improvements.
Participate in on-call rotation as needed.
This position is essential for enhancing the quality of care provided to patients while ensuring that their individual needs are met effectively within the healthcare system.
Qualifications
Required
High school diploma
2-3 years of scheduling experience in homecare, healthcare, or a related field.
Strong understanding of homecare operations, caregiver roles, and client needs.
Excellent communication and customer-service skills.
Ability to multitask in a fast-paced environment.
Strong computer skills (scheduling software, Microsoft Office, EMR systems).
Ability to remain calm, professional, and solutionfocused under pressure.
Preferred
Experience with Connecticut homecare regulations.
CNA, HHA, PCA, or healthcare background
Experience using homecare scheduling platforms (e.g.Sandata. ADACARE, Form DR).
