Pay: $20-$22.50 per hour, based on experience and language proficiency

Position Overview

Golden Years ADHC Lexington is seeking a culturally competent, multilingual Care Coordinator to serve our diverse seniors and families throughout Kentucky. The ideal candidate will demonstrate compassion, attention to detail, and a strong commitment to advocacy and wellness.

Responsibilities

Validate individualized care plans (POC) and supportive services for adult day health participants. Communicate fluently in English and at least one of the following languages: Swahili, Portuguese, French, or Arabic. Maintain uptodate records and documentation to support Medicaid regulations and center policies. Serve as point of contact between families, healthcare professionals, and community resources. Provide CPR and emergency support as a certified provider. Assist with scheduling, service navigation, crisis intervention, and family communication. Uphold the Golden Years mission and promote a respectful, inclusive, and supportive atmosphere.

Required Qualifications

High school diploma or equivalent; associate or bachelor's degree preferred. Proof of valid, current CPR certification (required). Fluency in English and at least one: Swahili, Portuguese, French, or Arabic. Minimum one year experience in care coordination, case management, or healthcare setting. Excellent communication, organizational, and interpersonal skills. Passion for personcentered care and multicultural engagement.

Compensation and Benefits

Competitive hourly wage: $20$22.50 per hour, commensurate with language proficiency and experience. Benefits package available including health, dental, paid time off, and professional development opportunities.
