The Advanced Practice Professional will work in collaboration with the providers of the Cardiology Department and Clinical Pharmacy team to provide outstanding, evidence-based, quality outcomes focused care in the outpatient Congestive Heart Failure Clinic. Types of job duties will include Chronic/Acute problem visits and Cardiology health maintenance associated with CHF.
This position will be a shift of Monday-Friday 8-hour shifts.
Responsibilities:

Provide medical services under the supervision of a physician Promote and maintain health Collaborate with physicians and other healthcare professionals Follow established standards, practices, and protocols
Requirements:

Graduate of an accredited Nurse Practitioner or Physician Assistant program Possession of an active TN state Registered Nurse and APN License with Certificate of Fitness to prescribe for APN's Medical knowledge Interpersonal and communication skills Patient care Professionalism Practice based learning Improvement systems-based practice
