7 am to 3:30 pm / Monday - Friday

General Summary of Duties:





Responsible for providing primary services, including assessing and managing patients in the clinic, following established standards and practices.

Essential Functions:




Incorporates evidence-based guidelines and performance measures in delivering clinical services and provides high-quality supervision of patient care. Appropriate and timely diagnoses based on findings of the current history and physical exam. Administer medications and injections in compliance with law and clinic guidelines. Performs procedures within the scope of practice. Instructs patient/family regarding medications and treatments. Educates patients regarding health and illness prevention. Recommends community resources to meet patient and family needs. Collaborates with physicians in managing acute and long-term medical needs of patients. Provides monitoring and continuity of care between visits. Maintains and reviews patient records, charts, and other pertinent information. Post-test examination results. Triages patient telephone calls and provides consultation. Properly documents all patient information and notes in the medical record. Medical records are completed and signed off in a timely fashion. Maintains patient confidentiality in compliance with HIPAA regulations. Supports initiatives such as Patient Centered Medical Home (PCMH) and Meaningful Use (MU) Participates and encourages diversity and inclusion within RVPCS. Other duties as assigned.



Requirements:




Current state RN license. Current APN license. Must be qualified in Basic Life Support Techniques. Participate in Quality Assurance Plan(s). Complete CME/Literature review as required.



Education/Experience:




MSN Required Successful completion of an approved Nurse Practitioner Program. 3 years of professional nursing experience in a clinic setting preferred. 2 years of Nurse Practitioner experience is preferred.



Skills/Knowledge/Abilities:




Skilled in applying and modifying the principles, methods, and techniques of professional nursing to provide ongoing patient care. Skilled in taking medical histories to assess medical conditions and interpret findings. Knowledge of how to use medical equipment and instruments to administer patient care. Knowledge of Infection Prevention. Ability to maintain quality control standards listed in the policy manual. Ability to react calmly and effectively in emergency situations.
