Job summary:



The Sterling Area Health Center (SAHC) is seeking a dedicated and experienced Advanced Practitioner (Nurse Practitioner or Physician Assistant) to join their primary care team in Hale, Michigan. SAHC is a Joint Commission accredited Patient-Centered Medical Home committed to providing high-quality, comprehensive, and compassionate primary care, dental, and behavioral health services to the entire community, with a special emphasis on the medically underserved. The ideal candidate will be a collaborative team player focused on delivering exceptional patient-centered team-based healthcare and improving patient outcomes.

Responsibilities:



Responsibilities include managing chronic conditions, acute illnesses, and minor injuries using evidence-based practices, and conducting physical examinations. The role involves identifying health risks, ordering and interpreting diagnostic tests, and developing and implementing patient care plans. Prescribing therapies, providing patient education, and performing minor procedures are also key duties. Advanced Practitioners will document patient care in the EMR/EHR (NextGen) system and work as part of a multidisciplinary health team, providing care across all ages in a family practice or walk-in clinic setting. Additional responsibilities may include leading population health initiatives and providing health education to the community.

Collaboration is essential, working effectively with physicians and other health professionals, and coordinating care with various providers and agencies. Serving as an educational resource for clinical staff is also part of the role. Participation in quality improvement reviews, professional development programs, and organizational meetings is expected. Additional duties could include providing pre- and post-operative care, performing emergency procedures and supervising staff.

Qualifications:



Education: Master's or Doctorate's degree from an accredited Nursing Program for NPs or PA Program for PAs. Experience: A minimum of 3,500 primary care patient hours or three years of primary care experience is preferred. Experience with EMR/EHR systems and in community/rural clinics, particularly Federally Qualified Health Centers, is also preferred. Licenses & Certifications: Active Michigan NP or PA license, active DEA license or eligibility, and CPR certification are required. The ability to be credentialed with SAHC's insurance plans is required. Skills and Abilities: Strong knowledge of primary care principles, practices, and pharmacology is needed. The ability to maintain quality standards, perform medical examinations, and communicate effectively with both professionals and the public is important. Key skills include educating patients, reacting calmly in emergencies, demonstrating excellent interpersonal skills, maintaining confidentiality, and being a strong team player focused on patient outcomes. Proficiency in using and maintaining electronic medical records is also required.
