The Telemed Expanded Advanced Practice Clinician (APC) will provide comprehensive medical services within scope of training for members of panel, regardless of age, religion, ethnicity or sex, on continuing basis. Using knowledge, medical instruments and equipment, provider will be capable of adequately diagnosing and treating a wide-range of acute and chronic diseases and injuries. The Expanded APC will have their own patient panel they will assume responsibility. They will also work closely with other physicians, consultants, advance practice providers and care service departments to facilitate patient health and well-being, increase patient and family communications, promote patient safety programs, and enhance the quality of clinical and programmatic outcomes. In addition to any contractual obligations owed by the provider to the organization, as well as any requirements established by SVH policies and/or procedures, the provider's duties and responsibilities include, but are not limited to the following job duties:




Essential Job Duties




Perform telehealth visits and record findings. Obtain complete medical history and physical data on patients.


Maintain appropriate medical records relating to all patients treated by him/her in a timely manner by eliciting and recording information about a patient's medical history.


Order or execute various tests, analyses, and diagnostic images to provide information on patient's condition, analyzing reports and findings of said tests and examination, allowing diagnosis of patients' condition. Coordinates transfers and consultations as appropriate.


Administers or prescribes treatments and medications. Promotes health by advising patients concerning diet, hygiene, and methods for prevention of disease. Immunizes patients for communicable diseases.


Ensure daily interactions represent the quality and experience expectations of SVH. Diligently work to uphold regulatory and compliance standards based upon the scope of practice. Intentionally coordinate efforts to ensure the practice is financially sound and meets productivity metrics. Actively participate in operational activities and flexibly adapt work to align with the strategic mission of SVH.


Complies with all policies, standards, mandatory training and requirements of Stormont Vail Health


Performs other duties as assigned


Qualifications




Must be a graduate of an accredited PA/APRN program. Meets educational preparation or other qualifications within specified time frames as determined/outlined by specific department policy. Required


5 years As a provider in a hospital and/or clinic setting. Preferred


Registered Nurse - KSBN Must have valid APRN licensure. Be certified as a nurse practitioner by a recognized national certifying body that has established standards for nurse practitioners. Required or


Physician Assistant (PA) - KSBHA Required


Additional certification may be required in certain patient care settings or populations. The department director will provide specific information and the time frame in which such certification may be attained at the time of hire/transfer into department.
