Position: Qualified Mental Health Professional - Older Adult Services

What You'll Do
As a Qualified Mental Health Professional, you'll provide direct treatment, case management, and mental health monitoring services to older adults with serious mental illness and/or intellectual and developmental disabilities. You'll play a key role in:
Conducting biopsychosocial assessments and developing person-centered plans
Coordinating care and linking individuals to essential community services
Monitoring progress and supporting recovery goals
Collaborating with nursing home teams to ensure mental health needs are met
Providing consultation and training to caregivers and community partners
Supporting OBRA Level I and II processes as needed
You'll be a trusted advocate, educator, and connector--helping individuals live with dignity, purpose, and support.
Compensation
BA (Non-Exempt): $23.89 - $30.99/hour
RN (Exempt): $27.72 - $38.27/hour
Full-Time Reports to: Older Adult Services Supervisor
What You Bring
Bachelor's degree in Social Work or related field, or RN license
Current Michigan licensure (LBSW or RN)
At least 1 year of experience working with older adults
Strong communication skills and a collaborative spirit
Functional computer literacy (within 6 months of hire)
Lived experience with mental illness, developmental disabilities, or substance use is valued
