to join our team at

Everett Transitional Care Services in downtown Everett

Schedule: Monday to Friday 1-2 days in facility and 3-4 days remote - total 30 hours per week
Pay Rate: $28 per hour

Admissions Functions:


1. Review the high volume of hospital and community referrals for potential placement and respond accordingly.
2. Provides information to resident applicants and/or representative regarding the admission process as needed via phone.
3. EMR kept current with pending admissions, including uploading clinical documents to their EMR.
4. Maintain a current referral log and resident waiting list in accordance with established policies and procedures.
5. Obtain necessary insurance pre-authorization for new/re-admissions.
5. Explain to the resident and/or representative room rates, billing procedures, visitors/guest privileges, restrictions, resident care procedures, etc., as appropriate via phone.
6. Coordinates all Admissions procedures with other involved departments and personnel.
7. Maintain confidentiality of all pertinent resident care information to assure resident rights are protected. Release information in accordance with established policies.

Qualifications

High School diploma or equivalent in education and experience. At a minimum, three (3) years of prior experience in long term care. Must be proficient with computer usage and programs like Microsoft Office Must be able to type at least 45 wpm and operate personal computer systems. Must possess the ability to make independent decisions when circumstances warrant. Must be able to read, write, speak, and understand the English language. Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel and professional health care personnel. Must have patience, tact, cheerful disposition, and enthusiasm, as well as be willing to incorporate them into existing practices.
