• Schedule: Full-time; Sun, Thurs, Fri, 9:00 a.m - 9:30 p.m.

Coordinate and facilitate patient admission according to patient acuity, bed availability, and required services.

• Perform onsite clinical assessments of potential candidates. Coordinate transfer process, working with referral sources, insurance companies, and various healthcare staff and departments.
• Monitor patient\xe2\x80\x99s insurance status during hospital stay.
• Assist with development and improvement of processes designed to keep referral sources informed of patient status and discharge recommendations.
• Provide pre-admission assessment, insurance verification, and pre certification for admission.

What You Will Need:
Licensure / Certification / Registration:

• Preferred Credential(s):
  
  • Registered Registered Nurse credentialed from the Illinois Board of Nursing.
  • Rehab Therapist credentialed from the Illinois Department of Financial and Professional Regulation.
  • Social Worker credentialed from the Illinois Department of Financial and Professional Regulation.

Education:

• High School diploma equivalency with 2 years of cumulative experience OR Associate\'s
  degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
