The Admissions Coordinator provides telephonic and in-person risk assessment to assist the physician in identification of appropriate level of care for potential Geri-psychiatric patients. The Admissions Coordinator is the initial contact with referral sources and potential geri-psych patients, coordinates the referral and intake process, facilitates the admission process, and maintains communication with referral sources, families, patients and unit staff. The admissions Coordinator is responsible for the verification and prior authorization of insurance benefits as needed.
Answers referrals and obtains all pertinent clinical, demographic, and insurance information to determine geri-psych patient's needs.
Collaborates with Utilization Management to ensure geri-psych patients are admitted into the appropriate level of care, insurance information is accurate, and authorization is obtained as appropriate.
Demonstrates knowledge of admission criteria for inpatient and outpatient levels of care and able to recommend appropriate service recommendations for each geri-psych patient. Provides referral recommendations to appropriate resources whenever a patient is not appropriate for services.
Schedules assessments with potential patients and referral sources and explains the assessment/admission process.
Conducts walk-in and scheduled assessments, coordinates with the reviewing psychiatrist, and makes appropriate clinical recommendations for the individual seeking care. Conducts assessments for pre-accepted patient transfers (direct admits) coming from hospitals and other approved referral sources.
Identifies potential problematic disposition cases, including placement problems, medical needs, or developmental delays, and seeks supervision prior to acceptance
Accurately and thoroughly enters information into the electronic medical records system in accordance with departmental protocol.
Education Requirements:
Graduate of an accredited program in Nursing (BSN)
Preferred master's degree Graduate of accredited program in Nursing
License or Certification Requirements
Current MS RN licensure, compact license.
Required 3 years clinical experience as a registered nurse in an acute care facility; Expertise in conducting psychosocial and risk assessments for admission to geri-psych/psychiatric care
