Reports to: Scotchman Living Center Director of Nursing


Develop and maintain an activity care plan for each resident and patient and integrate the activity care plan with other functional care plans. An Activity Coordinator plans, organizes, and leads engaging programs (like crafts, games, outings, fitness, etc.) to promote well-being, social interaction, and quality of life for participants (often seniors or individuals with disabilities) in settings of the nursing home and hospital, by managing calendars, budgets, volunteers, and ensuring activities meet diverse needs and safety standards. Acts in a professional manner positively representing Philip Health Services, Inc., while pursuing opportunities for personal growth.

Duties/Responsibilities:

Establish and maintain necessary records regarding activity attendance and activity care plans. Develop an activity care plan for each resident or patient and integrate the activity care plan with other functional care plans. Assesses each resident or patient for interests, talents, and previous patterns of time use and develops activity plans of care based on that assessment. Identify facility and community resources that can contribute to the activity care plan. Plans and schedules activities and develops a monthly calendar of activity events. Provides personal care to the residents and patients in a manner conducive to their safety and comfort. Helps with residents' hair care. Participate in leadership development process as defined by administration. Maintain professional qualifications by attending available in-service training sessions. Advises nursing, dietary, maintenance, volunteers, etc. of related responsibilities for an activity in advance, as necessary. Evaluates effectiveness of programs in terms of resident outcomes. Recruits and trains volunteer activity staff and maintains volunteer programs which utilize community involvement. Participate in leadership development processes as defined by administration. Evaluates effectiveness of programs in terms of resident outcomes. Promote program development by creating diverse calendars (arts, music, fitness, outings, social events) tailored to participants interests, abilities, and needs. Coordinate events by organizing, setting up, and facilitating activities, including booking entertainment, managing transportation for excursions, and supervisor volunteers.

Required Skills/Abilities:

Prior experience in a long-term care setting is preferred. Willingness to participate in weekend and holiday rotation as scheduled. Activity Coordinator must have the ability to adapt to frequent interruptions, the ability to identify and prioritize duties and the ability to work under stress and concentrate on critical tasks. Confidentiality must be maintained when working with medical, personnel, and financial information.

Qualifications/Education:

COTA or ability to obtain proper certification for Activity Coordinator Basic Life Support Certification or ability to obtain certification. Must be able to work effectively in a team-oriented environment with diverse groups of people and the ability to display a positive attitude and flexibility in changing situations. Must be able to communicate effectively with all team members within Philip Health Services, Inc.

Physical Requirements:

Very Heavy Work- Exerting more than 100 pounds of force occasionally, and/or more than 50 pounds of force frequently, and/or more than 20 pounds of force constantly to move objects. Prolonged periods of standing or walking are required. Substantial movements of the wrists, hands, and fingers. Must be able to use and manipulate hands to provide patient care. * Corrected vision and hearing within normal range to observe and communicate with patients and professional staff.
