What is an Assertive Community Treatment (ACT) team?





An ACT Team is a multidisciplinary group that provides comprehensive, community-based psychiatric treatment, rehabilitation, and support to individuals with severe mental illnesses. ACT teams offer services directly in the community, including medication management, therapy, crisis intervention, and assistance with daily living activities. The goal is to reduce hospitalizations and improve the quality of life by helping individuals live independently and achieve their personal recovery goals.

DUTIES AND RESPONSIBILITIES





Provide supervision of clinical services for the ACT Team.

Plan, implement and manage staffing to ensure required level of service to participants.

Facilitate team/staff meetings to encourage transparency amongst the inter-disciplinary team and provide clinical direction as appropriate.

Coordinate with ACT Team members and scheduling of all consultation meetings

Assist patients and staff with any telehealth related IT issues such as getting screens connected, troubleshooting IT and process issues when they arise, making sure patients have necessary information to connect to telehealth services.

Keep support staff updated on any schedule changes with patients or team members related to meetings and consultations.

Respond to emerging clinical and customer service needs in a prompt and professional manner.

Work collaboratively with other members of residential, care management, leadership and administrative staff to meet the customer service and clinical needs of participants and their families, including an on-call rotation with ACT Team members.

Monitor participant level of service and treatment provision by the team in accordance with guidelines for ACT Teams.

Ensure training and clinical supervision is provided within scope of practice.

Monitor the team/staff adherence to clinical documentation standards.

Review case records as to clinical appropriateness (i.e., clinical interventions are appropriate, follow-up services are documented, etc.)

Interview, hire, and train new staff members.

Provision of clinical services as needed.

Assist the program in developing policies and procedures.

Participate in CQI programming.

Treat all individuals with dignity and respect while maintaining agency standards of confidentiality, professionalism, and objectivity.

Maintain all HIPAA and 42 CFR confidentiality requirements and regulations as outlined in CARF policies and procedures.

Comply with agency policies, procedures, and regulatory standards.

All other duties as assigned.


QUALIFICATIONS/SKILLS/KNOWLEDGE REQUIRED FOR POSITION





Master's Degree in Human Services or Nursing preferred. If no Master's Degree, degree and equivalent training and experience will be considered.

Minimum of 3 years of clinical practice with knowledge of, and experience in, use of evidence-based practices.

Preferred experience working as part of an Assertive Community Treatment Team or other community-based mental health service.

Skill in group, individual and/or family counseling as appropriate.

Travel may be required as this is a community-based program.

Supportive of the mission of Prairie Ridge and the patients we serve.

Proficiency in oral and written communication skills, as well as documentation requirements.

Technology skills allowing for proficiency in use of electronic health record.

While performing the duties of the job, the employee is regularly required to sit; use hands to finger, handle, or feel objects, tools, or controls; and talk or hear. The employee frequently is required to walk and stoop, kneel, crouch, or crawl. The employee is often required to stand, reach with hands and arms, and climb or balance.

* The employee must infrequently lift and/or move up to 10 pounds and occasionally lift and/or more up to 35 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, and the ability to adjust focus.
